Phase II Cardiac Rehabilitation

Taking a step toward heart healthy living

Many individuals, just like you, have experienced a cardiac event. Don't let it hinder your life! Take this opportunity to take a step toward heart-healthy living with Cardiac Rehabilitation, and have some fun in the process!

For life-long cardiac health, it is important to begin cardiac rehabilitation as soon as possible after a heart attack or treatment of a heart condition. During the first phase of cardiac rehabilitation, which began in the hospital, you learned more about the factors that contributed to your heart condition, the first step toward a full recovery.

Now phase II continues your cardiac healing and rehabilitation in an outpatient setting close to home. The Upper Chesapeake Phase II Cardiac Rehabilitation Program provides a medically supervised, telemetry monitored, 12-week exercise and education program to help you successfully continue your recovery from a heart attack, bypass surgery, angioplasty, or other cardiac event.

The program is offered on an outpatient basis in our state-of-the-art fitness gym, conveniently located in the Ambulatory Care Center of Harford County, right in the heart of Bel Air.

There are three core components to the Upper Chesapeake Cardiac Rehabilitation Phase II Program.

1. Exercise Training

Exercise training is found to be the best lifestyle change you can make to improve your cardiac health. Your exercise training will be tailored for your specific needs and abilities.

  • Our specially trained cardiac rehabilitation team teaches you how to exercise safely
  • Your individualized exercise plan includes aerobic conditioning, flexibility, and strength training exercises
  • The exercise program is geared toward building your fitness level, strengthening your heart, and boosting your confidence

2. Education

Understanding your illness can lead to life-long, heart-healthy living. Our cardiac rehabilitation program brings together the expertise of cardiologists, cardiac rehabilitation nurses, exercise physiologists, dietitians and many other disciplines to help you and your family learn about heart disease, recover from your cardiac procedure and promote long-term cardiac health. You will learn to develop the skills necessary to make healthy lifestyle changes and make them part of your daily routine. Our in-depth cardiac education teaches you:

  • How to improve your diet: including what to eat and how to prepare it
  • Methods to lower your cholesterol through good nutrition and physical activity
  • Understand your medications: how to take them properly and what they are used for
  • How to quit smoking if you are a smoker
  • Important information regarding recovering from heart disease
  • How to recognize the signs and symptoms of cardiovascular disease
  • How to prepare to return to work should this be your personal goal
  • Strategies to reduce your risk of another cardiac event through risk factor modification
  • How to maintain good blood sugar and blood pressure control
  • Strategies to manage stress in your life
  • How to start and continue a safe exercise program, integrate regular exercise into your lifestyle, and tips for purchasing exercise equipment.

3. Support

Change is never easy. Our cardiac rehabilitation professionals will work with you to help you understand the changes and challenges you may face. We will teach you coping strategies that will help you come to terms with your heart condition and move on with your life.

OUR PROGRAM

You will get individual attention from our team of cardiac rehabilitation professionals even while you are exercising in a group setting. We limit our class sizes to maximize safety and fitness throughout the workout. To ensure heart-health during you exercise sessions, you will be electronically monitored through special wireless cardiac/heart monitors. The program typically lasts 12 weeks. Classes are three days a week and include approximately 1 hour and 45 minutes of exercise and education. We understand that family support can be a very large factor in recovery. We encourage family members to attend any, or all, of the education classes with you, if they choose.
